Insurance Guidelines

Vendors, contractors or service providers (hereinafter referred to as vendors) that do business with Sentry are required to provide an original Certificate of Insurance that meets or exceeds the established minimum requirements. The minimum requirements are determined by the type of good and/or service provided as well as the potential risk to Sentry. The coverage types and limits are to be considered as minimum requirements and in no way limits the liability of the vendors. If a product or service represents an unusual or exceptional risk, additional limits or insurance coverages may be required.

All active vendors are responsible for obtaining and attaching their applicable certificates of insurance in a pdf format and agree to keep the records current within the VendorNet site. Prior to the initial launching of this site as a service to our existing vendors, all existing COI information will be converted from the current system of record to the new database for vendors. If you are an active vendor, please verify that the information is accurate on the site when you set up your initial profile. Active vendors under current contract that do not meet the revised minimum requirements must submit the required coverages no later than the end of the current term of contract. All other vendors must comply to the revised requirements no later than the end of the current calendar year.

All prospective vendors should review the guidelines to identify all of the appropriate coverages and must indicate that each of the required coverages are in place for the type of good/service they are proposing to provide Sentry in the future and be prepared to provide certificates prior to the start of any sourcing activity. Prospective vendors may also elect to attach the required COI's at any time prior to the inception of any sourcing activities.

General Requirements

All renewal certificates must be provided in a timely manner, prior to the expiration date of the current certificate. A lapse in coverage and/or notice may jeopardize current and future business with Sentry. A 30-day notice of material change, alteration, and/or cancellation of the policy is also required.

The insurance carrier(s) listed on the COI must have an A.M. Best rating of "A" or better and have a surplus of at least $500,000,000 (financial size category of "X" or better). Ratings for the individual carriers can be verified by accessing the A.M. Best web site.

The required Certificates of Insurance are to be provided to Sentry Insurance prior to inception of the work or service being contracted. Renewal Certificates of Insurance will be provided to Sentry Insurance annually until completion of the work or service.

Coverage Descriptions

Listed below are coverages that may be required based on the type of business and the goods and/or services being provided to Sentry. Carefully review the descriptions listed below. If any of the coverage descriptions describe how you currently or propose to do business with Sentry, you are required to provide and attach the certificate(s) that meet or exceed the required coverages. The list of examples of goods and services provided is for reference only and does not represent the entire list of possible goods and/services.

Auto Liability: If driving a vehicle on Sentry's behalf as part of the work or service and if required to drive to and from a job location.
Minimum Requirement Limit: $1,000,000 Each Occurrence
Examples of Goods/Services provided: Delivery vehicles and freight, Construction, Repair and Maintenance, Consulting

General Liability: Required regardless of where the work or service is being performed. Provides coverage for Bodily Injury, Property Damage, Advertising Injury and Personal Injury (libel, slander, etc).
Minimum Requirement Limit: $1,000,000 Each Occurrence
Examples of Goods/Services provided: Required for all vendors.

Worker's Compensation: Statutory limits required by State where the work or service is being completed. If Worker's Compensation is not required (typically due to the number of employees), a Hold Harmless Agreement will be required to be completed and attached.

Professional Liability/Errors&Omissions: Additionally required if the vendor practices a profession which has a governing organization that establishes and maintains a set of standards and code of ethics and which has license requirements for its members. Professional Liability/E&O will also be required for certain vendors that may have influence over Sentry's ability to effectively operate its business and/or may put the company at risk of a lawsuit. Depending on the type of service, the minimum limit ranges from $1,000,000 to $2,000,000. Certain non-business critical services as defined by Sentry may qualify to be exempt from the Professional Liability requirement limits with a combination of General and Umbrella Liability that meets or exceeds the minimum requirement and will be reviewed on a case by case basis. This option also requires verification that no policy exclusions exist for the applicable occupation or service provided.

General: Consulting, Background Searches, Loss Control, Lab, Recruiters, Drug Testing, Background Searches, Relocation Services
Professional: Physicians, Attorneys, Accountants, Actuaries, Nurse Practitioners, Physical Therapists, Nurses
Finance Related- Auditors, Finance Consultants, Printers related to Finance
Claims Related- Bill Review, Arbitrators, Investigators, Adjusters, Appraisers
Real Estate- Landlords, Brokers/Realtors
IT-Consulting, Maintenance and Support

Cyber Insurance/Internet Liability: Additionally required coverage for vendors that operate as Application Service Provider/ Software as a Service. This coverage is for specific wrongful acts such as: infringement or unauthorized use of advertising material, copyright, slogan, trademark, etc through the internet; Failure to protect private or confidential information of others from unauthorized access on or through the internet; Making known to any person or organization material that violates a person or organization's right to privacy or publicity right; Plagiarism or unauthorized use of a literary or artistic format, character or performance through the internet; failure to prevent transmission of a computer virus to authorized users of a Web site or any private communication network such as customers, suppliers or supporters, on or through the internet.
Minimum Requirement Limit: $5,000,000 Each Occurrence
Examples of Goods/Services provided: Claims intake services, Retirement planning, MVR programs, Compliance information

Event Cancellation: May be additionally required for vendors who provide entertainment, speakers, and other services for special events.
Minimum Requirement Limit: $1,000,000 Each Occurrence
Examples of Goods/Services provided: Tournament Sponsors, Entertainment, Promoters, Agencies

Fidelity Liability/Financial Institution Bond: May be additionally required for vendors related to banking. Provides coverage for loss of money, securities, or inventory resulting from employee dishonesty, embezzlement, forgery, robbery, safe burglary, computer fraud, wire transfer fraud, counterfeiting, and other criminal acts.
Minimum Requirement Limit: Limit commensurate with risk
Examples of Goods/Services provided: Banking and Clearinghouse distribution

Garagekeepers Liability: May be additionally required for vendors who are involved with repair, service or storage of vehicles.
Minimum Requirement Limit: $1,000,000 Each Occurrence
Examples of Goods/Services provided: Auto and Glass Repair, Auto Transport

Liquor Liability: May be additionally required for vendors who manufacture, sell, distribute and serve alcohol beverages.
Minimum Requirement Limit: $1,000,000 Each Occurrence
Examples of Goods/Services provided: Beverage distribution

Medical Malpractice: Additionally required for vendors who are involved with providing health care.
Minimum Requirement Limit: $2,000,000 Each Occurrence
Examples of Goods/Services provided: Nurses, Nurse Practitioners, Physicians, Physical Therapists; Attorneys

Pollution Liability: Additionally required coverage for liability arising from pollution conditions caused by services or operations rendered by the vendor.
Minimum Requirement Limit: $1,000,000 Each Occurrence
Examples of Goods/Services provided: Fuel

Products/Completed Operations Liability: Additionally required for vendors who are involved in the distribution or manufacture of any product. Products/completed operations liability provides coverage for losses arising out of a manufacturer, merchant, or distributor's liability, due to injury or damage resulting from the use of a covered product and for damage or injury resulting from something the vendor made, repaired, or installed.
Minimum Requirement Limit: $1,000,000 Each Occurrence
Examples of Goods/Services provided: Retail inventory, Equipment rental, IT software, General Equipment, Telecom

Umbrella/Excess Liability: May be additionally required and provides coverage for liability claims that exhaust the underlying coverage limits provided by Auto Liability, General Liability, Employers Liability and other primary liability policies.
Minimum Requirement Limit: $5,000,000 Each Occurrence
